How can dermal fillers enhance facial contours?
- Reduce lines in the forehead and the eye area
- Help disguise nasolabial folds
- Reduce laugh lines
- Enhance the size and shape of your lips
- Fill hollow cheeks
- Enhance your chin and any sunken scars
It is important to note that dermal fillers are used to treat ‘static’ lines or wrinkle: this means lines you can see when your face is still. Also they can be used to enhance, or ‘plump up’, your lips, for soft, subtle results. Dermal fillers generally cannot help dynamic wrinkles including lines caused by muscle action (e.g. crow’s feet), very deep lines from sagging skin, neck lines and eye lines caused by eye bags.
Lips reach their fullness in our late teenage years or early twenties. Aging causes the lips to lose volume and become thin and flat. With sun damage the skin becomes less elastic and there is loss of lip edge definition, wrinkles and irregular pigmentation. The facial fat distribution decreases and combined with changes in muscle, loss of jawbone and teeth height result in loss of lip support. These support structures may occasionally need addressing to prevent or correct the down turning of lip corners.
In addition to lines, lost facial and cheek volume may also be a concern. Many women want to restore the lost volume from their cheeks that can leave them looking flat or hollow.
Types of Dermal Fillers
Restylane is a non-animal-based fully biocompatible hypoallergenic gel designed to smooth wrinkles, sculpt lips and shape facial contours. It is made of a natural substance that is found throughout your body. The most common areas for treatment are the glabellar lines (between the eyebrows) the nasolabial folds and for Lip and tear trough augmentation. The effects usually last for 4-6 months and further treatments maybe necessary. Restylane delivers a longer-lasting effect than bovine collagen.
The most commonly reported side effects were temporary redness and swelling at the injection site. These effects typically resolve within two to three days.
Juvédermis a non-animal-based highly biocompatible injectable gel similar torestylanethatgently reverses many signs of aging in the face, including fine lines and wrinkles, crow’s feet, thinning lips and loosening skin. It can also “plump up” the lips, sculpt the cheeks, and restore a youthful oval shape to the face. It is made with hyaluronic acid, a natural substance found in the body that works with collagen and elastin to give volume and support to the skin. Results last about 6-9 months and can be made to last even longer with a simple follow-up treatment a few weeks after the initial injection.
Juvederm is the first FDA-approved dermal filler which is a safe, effective, non-surgical option for those that want to diminish the appearance of moderate to severe facial wrinkles. The results make you look naturally younger, refreshed and can last up to one year.
Radiesse™ is a synthetic biocompatible filler made of Calcium Hydroxy apatite crystals, that reduces wrinkles, and scars by augmenting the body’s natural collagen supply. Radiesse is made from the same minerals found in our bones, so it carries little risk of allergic reaction. Radiesse can be used on many areas of the body, including the nasolabial folds, peri-oral lines, for depressed scars and defining the jaw line. By far its most used as treatment for reversings signs of ageing in the hand. Treatments typically take 15 minutes under Local Anaesthetic. Results are visible immediately after the injection and can last for months or years. Patients typically need only one or two treatments to achieve the full effect.
Sculptrais an hypoallergenic injectable made of Poly L Lactic Acid that corrects the signs of facial fat loss (lipoatrophy) by restoring lost volume under the skin. Lipoatrophy can result in sunken cheeks, indentations and hollow eyes. Injected into the deep dermis layer of the skin, it provides a gradual and significant increase in skin thickness, improving the appearance of folds and sunken areas. The results from Sculptra are not immediate but they are long-lasting. Effects usually last for up to two years after the first treatment session in most patients.Sculptra is FDA-approved for lipoatrophy correction in patients suffering from human immunodeficiency virus (HIV). It is the first FDA-approved product of its kind. Sculptra should not be used in patients who have active skin infections or inflammation (swelling) in the area to be treated, or who are allergic to any of its ingredients.
Hyaluronic acid (HA) is a polysaccharide that keeps the skin moisturized and gives it its elasticity. HA is present throughout our bodies, with 50% of it located in the skin. It occurs naturally in the deeper layers of our skin (the dermis), where it helps to keep skin smooth and ‘plump’ through its amazing ability to hold up to 1,000 times its weight in water. In a way HA is our skin’s own natural and internal moisturizer.Hyaluronic Acid in our bodies gradually decreases over time. In fact, 50-year-olds are estimated to have less than half the amount of HA they had in their youth. The drop in HA starts at around the age of 18 to 20 years and, by the age of 40, signs of ageing start to show—skin starts to lose its elasticity and volume as lines wrinkles and hollows start to appear. Injectable HA gel is a modified form of the HA that occurs naturally in your body.
Collagen is a protein substance that is the main structural support of skin, giving it strength. As part of the normal ageing process the skin’s natural collagen becomes depleted. Collagen replacement therapy adds support to your skin’s natural collagen to restore its resilience and strength and recapture its youthful appearance. Injectable collagen, filler helps minimize lines and scars, improves texture, and gives the skin a softer and more even tone.
HA vs Collagen are different substances but both are found naturally in your skin. Collagen helps provide your skin its structure and elasticity whereas HA serves to provide volume and internal moisturisation for your skin. HA filler smoothens your skin, helping to correct skin deficiencies and improving facial lines typically found between the eyes and around the mouth area. Collagen filler helps support the collagen network within your skin. By supporting the skin’s natural collagen, collagen filler helps minimise lines, improves texture, and gives the skin a softer and more even tone.
Because they play different roles in restoring your skin’s natural vitality, collagen and HA may be used together for the soft, subtle results you want. However, HA is now far more widely used in clinics because it can be administered at your very first appointment, whereas in order to be treated safely with collagen you will need to undergo an allergy test first.
As a naturally based biological material, HA is similar to your body’s natural HA and so does not require a skin test prior to treatment to test for allergic reactions.
The First Consultation:
Before the treatment, your skin type and medical history will be thoroughly evaluated. Your surgeon will discuss with you your goals and expectations from the treatment. Photographs will be taken for record keeping. You will need to sign a written consent form before the procedure.
Dermal Filler injection is performed as an Outpatient procedure. The procedure is essentially painless. The area for injection is prepared by administering a topical anaesthetic cream. Occasionally a local nerve block may be given. The filler is injected into the area with a very fine gauge (30G) needle Typically HA and collagen procedures may last 10-20 minutes depending on the number of areas for injection.
After the procedure you should recover almost immediately and be ready to return home. Some bruising and swelling may occur and possibly last from 2-5 days. Any swelling can be reduced by using ice soaks over a 10 minute period every 4 hours for the first few days.
Most people find that they can return to work or go about their normal daily activities immediately after treatment. In general, the effects of treatment with temporary fillers last from 4 to 5 months.
After treatment with dermal fillers, you should notice visible results immediately. The lines on your face will appear minimised and smoothed out. If your treatment plan included lip enhancement, you will notice that your lips appear fuller and more defined. If cheek voluminizing has been done you will notice that your cheeks look plumper and your face looks younger.
You will be asked to come back after about two weeks for a check up. Occasionally a little top-up is needed to ensure you are getting the look you desire.
Follow-up treatments are usually required to maintain the effect usually at 6 months. Collagen treatment effects last three to five months. When repeat treatments occur over time, either the interval between appointments becomes longer, or less product is required to achieve the same results, or more areas can be treated with the same amount of product.
