Breasts are the ultimate symbol of female beauty. For many women, breast size is an important part of feeling desirable and feminine. Breast augmentation to increase fullness and projection of the breasts balance a woman’s figure, enhance her self confidence, as well as can drastically improve the way she views herself.
Also known as Augmentation Mammaplasty, breast augmentation enlarges a woman’s breasts through the surgical placement of breast implants. Permanent breast enlargement can only be achieved surgically. There is currently no medical treatment of any kind for breast enhancement with the proven result and satisfaction of breast augmentation. The aim is to tailor your breast implant procedure to suit your body and your goals. Dr. Naithani firmly believes that your breast implant surgery should be focused on maximizing aesthetics and minimizing your return to work.
Fat grafting to augment the breasts has gained in popularity in recent times, where patients own body fat is used to augment the breasts.
Am I the Ideal Candidate for a Breast Implant ?
Breast augmentation is designed to improve the size, shape, and overall appearance of the breasts with aesthetic, natural looking results. Besides its cosmetic benefits, the surgery can also create a positive psychological impact by renewing your self-confidence and esteem in yourself. Breast enhancement through the placement of implants will benefit you if you feel that:
Your breasts have lost volume following pregnancy
Your breasts have become unshapely and reduced in size following weight loss
Your breasts are asymmetrical
Your natural breast size makes you look less feminine
You feel self-conscious about your body in a bathing suit
Breast augmentation is a highly individualised procedure- one that you should choose to undergo to fulfill your own desire for enhanced breast size, not the desire of others.
Saline Breast Implants
These are approved by the U.S. Food and Drug Administration for females who are at least 18 years of age. Saline implants consist of a saltwater (saline) solution covered by an outer silicone shell. These are frequently placed behind the muscle to give a natural look. Saline implants have lower risk of implant hardening, and result in a much smaller scar due to ease of insertion through a smaller incision.Saline implants however have a higher incidence of wrinkling and the feel is also not as natural. They are best avoided in women with saggy small breasts.
Cohesive Silicone Implants
Cohesive silicone implants are FDA-approved for breast augmentation in patients who are 22 or older, as well as for breast reconstruction patients of all ages. 4th generation cohesive silicone implants are usually round in shape with thicker shells with more cohesive silicone gel than the breast implants of previous generations. The main advantage of the newer generation implants is a look and feel which is more natural with negligible rippling.
Highly Cohesive Silicone Implants
Highly cohesive silicone implants are composed of a durable, form-stable silicone gel intended to closely mimic the feel of natural breast tissue and are available in round and anatomical varieties. The anatomical shaped implants yield mild fullness in the upper pole and more substantial volume in the lower pole of the breast. They are generally thought to be more resistant to issues like rippling and leakage, and are an excellent option for patients with a thinner body frame. Since the FDA reapproved the use of the silicone gel implants in 2006, the mood has shifted greatly in favor of silicone gel due to their natural feel and long-lasting integrity.
Anatomic-Shaped, Form-Stable Cohesive Gel Implants
Anatomic-shaped, form-stable cohesive gel breast implants, also known as “gummy bear” or “tear drop” silicone gel implants, are the newest implant devices to be FDA approved in the US. These implants have been widely used and documented in Europe for a long time, and therefore are largely viewed as the implants giving the most natural look to the breast. Once placed, these progressive-style implants are known to better retain their shape, and clinical studies have shown a reduced risk of rippling and capsular contracture.
There are two shapes of breast implants available for breast augmentation, round and anatomical implants. The type of implant used is made based on the patients desired goals, and is also dictated by the amount of breast tissue present.
If you desire more fullness in the upper pole of your breast, a round implant will be preferable. If a more natural appearance is required with more volume toward the lower pole below the nipple, an anatomical shaped implant is better.
Selecting the right Breast Implants
At the beginning of your consultation, Dr. Naithani will understand your specific needs and desired outcome for Breast Enhancement surgery. All options will be discussed in thorough detail including the advantages and disadvantages of each choice. Next a physical exam will be done along with chest and breast measurements to know what size implants will best suit you according to present breast size and body proportion. This includes a thorough evaluation of your current breast size, position, shape, symmetry and overlying skin elasticity.
Once you have an understanding of all options, Dr. Naithani will help you select the breast implants that will mostsuccessfully help you realize your goals for breast augmentation. For this scientific 3D measurements of the breasts and shest is done and all values gathered are then converted into the most suitable Implant size. Most patients request for a particular cup size not knowing that there is no standard for bra sizing. 36C in one company maybe a 34D in another, generating confusion about cup size. Another great aid for patients confused about selecting the suitable implant size is the use of sizers. The most important thing which will defines a happy patient is if you like what you see in the mirror with the sizers. It is advisable to wear form fitting tops and a nonpadded bra to try on the sizers.
It is important to note that the look of saline and gel implants is similar. The difference is in how they feel. For women who do not like the idea of silicone in their body saline implants are a better choice. For those who want a truly natural feel to their breasts gel implants are preferable. Silicone implants are also preferable to many due to their lower risk of rippling.
It is advisable you get mentally used to seeing yourself with bigger breasts. To avoid the “I wish I would have chosen a little bit bigger” phenomenon, stuff your bra for a few days or week before surgery. Test out and get comfortable seeing yourself withyour new size.
At home the RICE TEST is most helpful in defining size.
At the same time a thorough medical history will be reviewed, and a physical examination done to determine if you are a good candidate for breast implants.
Your Breast Augmentation Surgery
Breast augmentation is usually done under general anaesthesia in a hospital. The implants are commonly inserted through a tiny incision hidden in the fold below the breasts called the inframammary fold. .
Other incisions such as the peri-arolar and axillary can also be used though none give the access and accuracy of implant placement as the Inframammary one. The inframammary incision is of about 4.5 cm length. A implants are placed in a pocket is created under the breasts. This pocket will either be under the chest pectoralis major muscle or above it, depending on the pre existing volume and position of your breasts.Dr. Birraj utilizes a minimal touch technique with silicone gel implants that minimizes any risk of contamination, implant shell integrity loss, and therefore capsular contracture
Dr. Naithani prefers to place the implant in the subglandular position, however the placement is generally tailored to individual needs. Submuscular implant position is recommended If the lower half of the breast has less breast mass.
In some cases a dual plane subpectoral breast augmentation may need to be done specially in patients that have early degree of breast ptosis or “droopiness” to the breast. This allows some degree of “internal lift” to be accomplished in conjunction with the breast augmentation. However in some cases of moderate breast droopiness breast implants may need to be done alongside a Breast Lift or mastopexy.
The wound is closed meticulously. A tube to drain out collected blood may or may not be placed. You will notice the difference in the size of your breasts almost immediately. They will be strapped in a firm compressive dressing. As the wounds are closed with absorbable sutures, stitch removal is not usually necessary.
Breast augmentation is a very safe surgical procedure that takes a little over an hour to perform. Patients are generally discharged the same day under the supervision of a family member. Risks of surgery are minimal. In the occurrence of a postoperative hematoma, early recognition and treatment must be carried out to correct the problem.
Recovery following breast augmentation, takes about 2 -3 days. Patients are sent home on a prescription of antibiotics pain relievers and muscle relaxants. The patients are seen again on the 3rd post op day for change of dressing and are made fitted into a light front zip sports bra. Patients can take a shower after the first dressing change. Majority of patients are able to resume normal daily activities in a day or two. Return to moderate exercises or weight lifting is encouraged after 3 weeks to ensure safety and minimize any risk of complications.Once the swelling has subsided, it takes 3-4 months for the breasts will settle to a more natural and pleasing location and shape.
Breast Augmentation is typically done via a small inframammary incision hidden under the breast, just over 1.5 inches in length. This incision heals extremely well has, is almost invisible, has less chances of capsular contraction, and less nipple sensation
Initially all breast implants have an operated firm and in your face look. The implants will “drop” into place as the swelling recedes over the coming weeks/ months. Patientsare adviced for implant massage or exercises to help facilitate the implants falling into place.
Patients can resume swimming hot tub etc at least a month after their surgery. Initially all breast implants have an operated firm and in your face look. It is inadvisable to undertake these activities before that as chances of infection occurrence are higher.
Most patients will experience some transient increase or decrease in nipple sensitivity due to the “expansion” of the breast, which resolves over several months.
Yes. Breast augmentation, when performed by a skilled and experienced plastic surgeon, does not affect a woman’s ability to breast feed her child.
Many schools in plastic surgery tell their patients that saline implantswill need to be replaced every 10 years. In my view if there are no concerns they only need to be changedif the patient wants to change them. Saline deflation rates are typically about 1% per year. It is usually spontaneous and not related to trauma. Silicone gel implants have been used since the early 1960s, and it is well known from history that even with the older generation of implants, they often would last 20 to 30 years. The current 4th or 5th generation silicone gel implants have better implant shell designs as well as a more cohesive gel that will be less susceptible to leakage or rupture.
Women can safely undergo mammography after breast implants, including both saline and silicone. An MRI maybe indicated in cases where the breast implant may obstruct the view of the breast tissue. Your radiologist must be informed that you have implants prior to your mammogram so they are properly prepared for your visit.

The UK Government appointed Independent Review Group (IRG) July 1998 has concluded that there is no conclusive evidence for any link between use of silicon and immunological or connective tissue disorders. Recently USA FDA has also approved use of silicone implants manufactured by Mentor.Silicone is a compound made from silicon, a natural chemical element. Silicones have been used in moisturising creams, suntan lotions, lipsticks, medicines, food preparation and many other everyday products for more than 50 years.
Textured implants have been rarely associated with the rare ALCL cancer. Statistics show this to be as rare as 33 in 1 million patients.
Implants do not affect screening of the breast, but they may make it technically difficult and special views may be necessary. Hence, the technician should be informed prior to screening.
