Saline Breast Implants

These are approved by the U.S. Food and Drug Administration for females who are at least 18 years of age. Saline implants consist of a saltwater (saline) solution covered by an outer silicone shell. These are frequently placed behind the muscle to give a natural look. Saline implants have lower risk of implant hardening, and result in a much smaller scar due to ease of insertion through a smaller incision.Saline implants however have a higher incidence of wrinkling and the feel is also not as natural. They are best avoided in women with saggy small breasts.

Cohesive Silicone Implants

Cohesive silicone implants are FDA-approved for breast augmentation in patients who are 22 or older, as well as for breast reconstruction patients of all ages. 4th generation cohesive silicone implants are usually round in shape with thicker shells with more cohesive silicone gel than the breast implants of previous generations. The main advantage of the newer generation implants is a look and feel which is more natural with negligible rippling.

Highly Cohesive Silicone Implants

Highly cohesive silicone implants are composed of a durable, form-stable silicone gel intended to closely mimic the feel of natural breast tissue and are available in round and anatomical varieties. The anatomical shaped implants yield mild fullness in the upper pole and more substantial volume in the lower pole of the breast. They are generally thought to be more resistant to issues like rippling and leakage, and are an excellent option for patients with a thinner body frame. Since the FDA reapproved the use of the silicone gel implants in 2006, the mood has shifted greatly in favor of silicone gel due to their natural feel and long-lasting integrity.

Anatomic-Shaped, Form-Stable Cohesive Gel Implants

Anatomic-shaped, form-stable cohesive gel breast implants, also known as “gummy bear” or “tear drop” silicone gel implants, are the newest implant devices to be FDA approved in the US. These implants have been widely used and documented in Europe for a long time, and therefore are largely viewed as the implants giving the most natural look to the breast. Once placed, these progressive-style implants are known to better retain their shape, and clinical studies have shown a reduced risk of rippling and capsular contracture.
There are two shapes of breast implants available for breast augmentation, round and anatomical implants. The type of implant used is made based on the patients desired goals, and is also dictated by the amount of breast tissue present.

If you desire more fullness in the upper pole of your breast, a round implant will be preferable. If a more natural appearance is required with more volume toward the lower pole below the nipple, an anatomical shaped implant is better.

Your Breast Augmentation Surgery

Breast augmentation is usually done under general anaesthesia in a hospital. The implants are commonly inserted through a tiny incision hidden in the fold below the breasts called the inframammary fold. .

The patients are seen again on the 3rd post op day for change of dressing and are made fitted into a light front zip sports bra. Patients can take a shower after the first dressing change. Majority of patients are able to resume normal daily activities in a day or two. Return to moderate exercises or weight lifting is encouraged after 3 weeks to ensure safety and minimize any risk of complications.Once the swelling has subsided, it takes 3-4 months for the breasts will settle to a more natural and pleasing location and shape.

It is usually spontaneous and not related to trauma. Silicone gel implants have been used since the early 1960s, and it is well known from history that even with the older generation of implants, they often would last 20 to 30 years. The current 4th or 5th generation silicone gel implants have better implant shell designs as well as a more cohesive gel that will be less susceptible to leakage or rupture.

Women can safely undergo mammography after breast implants, including both saline and silicone. An MRI maybe indicated in cases where the breast implant may obstruct the view of the breast tissue.
